PLEASE READ SCHOLARSHIP TIPS BEFORE APPLYING
We are pleased to provide an online form for you to apply for a PROWUS Scholarship. We hope this makes the application process easier for you.

IN ORDER TO COMPLETE YOUR PROWUS SCHOLARSHIP APPLICATION YOU WILL NEED TO:
A. FILL OUT ALL INFORMATION REQUIRED ON THE APPLICATION FORM BELOW, AND
B. PROVIDE THE FOLLOWING DOCUMENTS TO PROWUS:
1. PROOF OF INCOME. Acceptable forms include ONE of the following:
The first two pages of last year's IRS 1040 tax return (please block out Social Security Numbers and birthdates)
Copies of paystubs for the past 2 months (for all supporting parents)
A qualifying letter of enrollment in Federal Free / Reduced Lunch Programs*
2. TRANSCRIPT of current semester grades.
3. ESSAY (written or video) from applicant.
4. LIST OF REFERENCES: A minimum of two references are required: one from an instructor, one from a teacher, coach or mentor.

5. REFERENCE FORM: To be filled out by people on your reference list; this is to be sent directly from the reference to the PROWUS Grant Committee.
